No. We do not charge for membership.

Yes. A Special Olympics physical is required prior to the start of training. This does not have to be done by your primary physician.

In order to be eligible for participation in Special Olympics, every athlete must have a valid Medical Form (previously Med App) on file with Special Olympics Illinois prior to the start of training. Special Olympics Illinois only accepts its own form, forms from other states are not accepted.

The Medical Form is valid for 3 years from the date of the examination regardless of the parent/guardian/entrant signature date. The Medical Form cannot be altered in any way (information crossed out, wording added that alters any of the waivers, etc.)

If an athlete had a valid Med App on file prior to the introduction of the Medical Form, the Med App remain valid until its expiration date, however, the athlete will need to submit a Consent Form prior to participation. An athlete’s parent/guardian and/or the athlete, if an adult without a guardian, is solely responsible for the accurate completion and timely submission (prior to any relevant deadline*) of the Medical Form to Special Olympics Illinois. Please submit completed Medical Form to your agency’s Special Olympics Athletic Director or head coach. You may also submit a Medical Form direct to your Region office. Failure of the athlete to have an accurate, fully completed Medical Form on file with Special Olympics Illinois prior to any relevant deadline* will result in the athlete being denied participation in Special Olympics Illinois competitions, programs and events. Medical Form must be valid throughout the completion of the competition, program or event.

Special Olympics Illinois strongly suggests that all athletes update their Medical Form annually during their annual physicals in order to make certain they are able to participate in all Special Olympics Illinois competitions, programs and events.

A parent/guardian has several options to determine a Medical Form expiration date:

  • Use the Athlete Lookup option
  • Contact your agencies Special Olympics Athletic Director or head coach. Special Olympics Illinois provides expiration date information to these individuals on a consistent basis.
  • Contact your Region office to inquire about the expiration date.
  • * Relevant deadline will most likely be sooner than the Medical Form expiration date.

The Consent Form is valid for the lifetime of an athlete, barring change in guardianship. New athletes will need to submit their Consent Form with their first Medical Form. Renewing and returning athletes will need to submit a consent form prior to their participation regardless of their Medical Form status.

Athletes will only need to submit a new Consent Form again only if/when their guardianship changes: when they become their own guardian (usually age 18), or their legal guardian person or contact information changes.

Yes. If you choose to come and watch or help your athlete(s) bowl, you're more than welcome to join us.
